Patient Access Clinical Coordinator

We’re looking for a Patient Access Clinical Coordinator who shares this vision.
Your Opportunities
- Reviews referrals to hospice for Home Care or Angela Hospice residence services.
- Conducts inpatient residence reviews for appropriateness and collaborates with nursing team for a successful admission.
- Collaborates with referral sources to participate in transitions of care by obtaining information, arranging admissions, ordering DME, and being a clinical resource for patients and families during the referral process.
- Assists the Director of Patient Access with providing clinical support to the Patient Access Coordinators, Patient Family Liaisons, and Admissions Nurses.
- Assist with facilitation of sign-on paperwork for patient and families in the inpatient setting.
- Charts all pertinent information in an electronic medical record.
- Reviews and follows up on phone calls taken overnight and on weekends.
- Prepares the discharge for patients with pumps, including coordinating the IV company, with family and assigning RN to set up.
- Notifies RN/MSW regarding family calls.
- Confirms and/or corrects in computer last home address, current location, and contact for patient as needed.
- Orders home care supplies for patients, equipment, durable medical equipment (DME), as needed.
- Collects intake information, as needed.
- Orders ambulance and enters as order in EMR, orders equipment and IV therapy, as needed.
- Notifies discharge planner at hospital of admission time/date confirmation.
- Works closely with Patent Access Coordinators to schedule same day admissions to improve length of stay.
- May complete an admission if the need arises.
What You Bring
- Graduate of an accredited school of nursing
- Current Registered Nurse license in the State of Michigan
- Certified Hospice and Palliative Nurse preferred
- CPR Certified
- Hospice experience in an inpatient or home care setting preferred
- Triage nursing experience preferred
- Excellent organization and time management skills
- Proficient computer knowledge and skill
- Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist
- Ability to work with little or no supervision
- Capacity to lift, position and/or transfer patients
- Auditory and visual skills
- Proficient interpersonal relations and communicative skills
- Knowledge of Federal/State, NHPCO, and CHAP regulations and guidelines
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
