Salary: $62,400 annually

Your Opportunities

Comfort.

  • Bring a sense of calm and comfort while giving support and counseling to patients and their families, as well as assisting with crisis intervention when needed.
  • Play a key role in the development of treatment for every patient and family, serving as both a consultant and team member who is actively involved in providing direct services.

Advocate.

  • Utilize existing community resources to better accommodate the needs of patients and their families, including referring them for financial assistance if there’s a need.
  • Act as a liaison between social agencies and the Angela Hospice team.
  • Be there for families and patients if they wish to develop an advance directive.
  • If need be, provide families with guidance for funeral planning.

Connect.

  • Complete patient admission procedures, such as initial biopsychosocial assessment, and creating a care plan.
  • Keep patients’ records and documentation up-to-date.

Collaborate.

  • Work with members of the Interdisciplinary Group (IDG) to help them better understand the significant social and emotional factors related to patient care.
  • Act as a resource for hospice team members for services available in the community that can be used to meet the psychosocial/economic needs of patients.

What You Bring

  • Graduate of an accredited institution, MSW or equivalent education/training required from an accredited in social work and bereavement services
  • Current licensure in the State of Michigan, LMSW, LLMSW
  • Previous hospice experience preferred
