Reports to: Patient Support Center & Telehealth Clinical Manager
Salary: $36/hour

We’re looking for a Clinical Coordinator/On-Call RN who shares this vision.

Your Opportunities

  • Coordinate, implement and evaluate the delivery of patient services
  • Chart patient/family phone calls/triage
  • Assign admissions to RN as requested by Patient Access Center
  • Order medications, chart that information and notify RN Case Manager
  • Arrange and assign transfers and tuck-in visits
  • Review and follow up on phone calls taken overnight and on weekends
  • Prepare the discharge for patients with pumps, including coordinating the IV company with family and assigning RN to set up
  • Notify RN/MSW regarding family calls
  • Arrange tuck-in/admission visit packets and collect materials for patient transfer from hospital to place of residence
  • Assign RN/MSW to death or emergent visits or attend visit yourself PRN
  • Arrange special procedures at the hospital
  • Confirm and/or correct in computer last home address, current location and contact for patient
  • Discern appropriate discharge planning for transfers
  • Arrange emergent GIP or Continuous Care change of status for patient in crisis at home with supervisor assistance
  • Order home care supplies for patients, equipment, durable medical equipment (DME), as needed
  • Communicate with patients, families and caregivers via Angela Hospice provided phone, secure messaging, secure emailing, and telehealth platforms as requested
  • Collect intake information as needed
  • Order ambulance and enter order in EMR, order equipment and IV therapy, as needed
  • Notify discharge planner at hospital of admission time/date confirmation
  • Maintain report of admissions, deaths, changes, and imminent patients at end of each day including ID number, acuity, status and city
  • Provide clinical support to professional staff on assigned team in the delivery of patient care
  • Work closely with Patient Access department to schedule same day admissions to improve length of stay
  • Make home visits or do an admission PRN

    What You Bring

    • Graduate of an accredited school of nursing
    • Current license in the State of Michigan
    • Certified Hospice and Palliative Nurse preferred
    • CPR Certified
    • Hospice experience in an inpatient or home care setting preferred
    • Triage nursing experience preferred
    • Excellent organization and time management skills
    • Proficient computer knowledge and skill
    • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist
    • Ability to work with little or no supervision
    • Capacity to lift, position and/or transfer patients
    • Auditory and visual skills
    • Proficient interpersonal relations and communicative skills
    • Knowledge of Federal/State, NHPCO, and CHAP regulations and guidelines
    • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
