Your Opportunities
- Develops, implements and maintains an effective, ongoing, hospice-wide data-driven Quality Assessment and Performance Improvement Program that reflects the complexity of its organization and services, involves all hospice services (including those services furnished under contract or arrangement), focuses on indicators related to improved palliative outcomes and takes actions to demonstrate improvement in hospice performance
- Tracks and benchmarks hospice and organizational trends by following and compiling data from both internal and external sources and prepares reports for the leadership team as necessary
- Oversees the grievance program by ensuring tracking, trending, maintaining and resolution of grievances
- Develops and provides education and support of staff in quality improvement initiatives, providing hands on education and training, to include different learning modalities (i.e., classroom and in person/onsite trainings)
- Assists in the assessment of workforce learning needs and knowledge gaps using both internal resources (i.e., audit and/or investigation results, data analysis, and reported deficiencies) and industry identified high-risk areas
- Manages, supports and drives implementation of approved best-known practice to include visit design, phone interaction, and standardized documentation. (Includes planning, training, competency assessment and performance monitoring of team members)
- Conducts field observations, coaching and reporting
- Identifies and reports on quality metrics
- Provides direction and coordination of quality improvement activities utilizing continuous quality improvement principles and methodologies
- Works collaboratively with all staff to assist in implementing strategies, actions and process improvement ideas that strengthen the delivery of patient care services and organizational compliance with laws, regulations, and organizational policies
- Participates in development of effective policies and procedures following CMS and State guidelines, while promoting patient –centered, holistic care approaches
- Collaborates with Quality/Education team and leadership to oversee survey readiness activities for CHAP, State, Federal and any other survey activity Angela Hospice is subject to
What You Bring
- Valid nursing license in the State of Michigan preferred, BSN or MSN preferred, CHPN preferred
- Experience in hospice, home care, acute, sub-acute, or long-term care required
- At least two years of active experience in developing and managing comprehensive quality and compliance programs
- Proficient in Electronic Medical Record platforms
- Excellent problem-solving skills that are rooted in both quantitative and qualitative assessment
- Must be capable of maintaining a high degree of efficiency and accuracy, be motivated, energetic and resourceful
- Experience working in a team-oriented, collaborative environment
- Proven ability to train and educate others
- Superior verbal and written communication skill
- Strong computer skills (Word, Excel, PowerPoint, Sharepoint and Outlook)
