Hospice Nurse Practitioner

We’re looking for a Hospice Nurse Practitioner who shares this vision.
Primary Duties and Responsibilities
- Responsible for examination of patients to determine diagnoses, to treat diseases and to provide pain and symptom management.
- Performs therapeutic management including, but not limited to, care planning, providing prescriptions, coordinating consultations and referrals.
- Provides an environment to support the patient and family’s emotional needs.
- Provides appropriate education to the patient and family taking into consideration their current knowledge base.
- Creates an atmosphere of honesty, compassion, respect by being physically, as well as, mentally available for the patients and families.
- Works collaboratively with the Angela Hospice clinical team, as well as, augments teams in hospital, nursing home and assisted-living facilities.
- Assumes responsibility for compliance with federal, state and local regulations.
- Presents an appropriate, professional appearance in accordance with established policy.
- Demonstrates dependability and punctuality.
- Accepts assigned duties in a cooperative manner.
- Works cooperatively with patients, families, nursing staff physicians, consultant personnel and ancillary service providers.
- Performs other duties as directed by the Medical Director or his/her designee.
What You Bring
- Master’s Degree from an accredited school
- Nurse Practitioner License in the State of Michigan, graduate of an accredited Nurse Practitioner program
- Minimum of five (5) years direct patient care experience required; Hospice and Palliative Care experience preferred
