NOTICE OF PRIVACY PRACTICE

Angela Hospice Home Care, Inc., takes the privacy of your health information seriously. Angela Hospice Home Care, Inc., is required by law to maintain that privacy and to provide you with this Notice of Privacy Practices. This Notice is provided to tell you about our duties and practices with respect to your information. Angela Hospice Home Care, Inc., is required to abide by the terms of this Notice as are currently in effect. Many parents are unsure about providing camps with information about personal aspects of their child’s behavior or past experiences. Some patients fear that the information may be misused, while others are concerned about their child being “labeled,” singled out, or treated differently. All parents want to see their child have a good start at camp, unencumbered by past problems.

At Camp Monarch we appreciate these concerns. We also know how valuable such information canbe in assisting us to help your child make his or her adjustment to camp as smooth as possible. Very often having prior knowledge about a learning difficulty or a bed-wetting problem can be the crucial factor in helping us be sensitive to your child’s need for patience, understanding, and reassurance. Since children often automatically use their behavior rather than words to tell us what’s been bothering them, having advanced knowledge of areas that might be difficult for your children really helps us understand the message in their actions.

Our commitment is never to misuse such information or to release it to unauthorized persons. It will never be used at camp unless necessary, and then only with the greatest of discretion. If you wish, we will inform you of any need to share this information with the counselors who are most in contact with your child, and will certainly let you know if your child is having difficulty.
